Appointments & Visits
There are several options for scheduling an appointment.
For New Patients, please call 704-862-4700 or complete online referral form or have your physician send in a referral.
Current patients (if you have been seen within the last three years), please complete online form or contact your clinic directly.
A copy of your insurance card, valid ID, list of any medications, previous MRI/CT/Xray, previous pain clinic records.
Insurance & Billing
Yes, SEPSC will file your primary and/or secondary insurance to verify what portion of your visit they will cover. Any remaining balance will be billed to the patient.
